VAN WERT, Oh. (WOWO):  Charges have been filed after a fatal September traffic accident.  

38 year old Babette L. Stewart of Van Wert has been charged with Vehicular Homicide, a first degree misdemeanor, as well as Vehicular Manslaughter, a second degree misdemeanor.  

The accident happened on September 23rd at the intersection of State and Mendon in Van Wert County.  George E. Brake died as a result of the accident.  

Stewart is scheduled to appear in court December 21st.
