FORT WAYNE, Ind. (WOWO): We now know the identity of a man shot in the face last Sunday night. It’s Jesus Chavez, and just before midnight he drove to a local hospital and checked himself in. Our news partners, 21Alive, say Chavez arrived in a vehicle with broken windows, bullet holes and blood stains.  It’s believed he was shot on Greenwood Avenue.
