PERRYSBURG, OH (WOWO) A 65-year-old Toledo police lieutenant, Adam Fish, has been indicted on a third-degree felony for aggravated vehicular assault and two first-degree misdemeanor charges for operating a vehicle under the influence, according to court records and reporting by WTOL 11.
The crash occurred December 20 on State Route 25 at Roachton Road in Perrysburg. Police say Fish was leaving a work shift party when his vehicle struck another car, triggering a three-vehicle collision.
Officers reported Fish had glassy eyes, slurred speech, and the smell of alcohol on his breath. Fish initially refused a field sobriety test but later completed one, failing several components. He also refused a breath test, prompting a warrant for a blood draw. Test results have not been released.
Fish has been with the Toledo Police Department since 2011 and was promoted to lieutenant in July 2025. Body and dash camera footage related to the crash has not been released due to the ongoing investigation. Attempts to reach Toledo police for comment were not returned at the time of publication, WTOL 11 reports.
