INDIANAPOLIS (AP) _ A new state report says a lack of supervision is a factor in most children drowning deaths in Indiana waters.
 
  The Indiana Department of Child Services and the Department of Natural Resources issued the report Tuesday on the 11 youth drowning deaths in fiscal 2011, the last year for which complete statistics are available.
 
  Child agency director Mary Beth Bonaventura says it is the responsibility of adults to make sure children are safe in water.
 
  The DNR says three children have been among Indiana’s 27 drownings this year. That includes a 5-year-old boy found in a northeastern Indiana lake 25 miles north of Fort Wayne last week. Conservation officers say family members noticed Dominic Slone-Lehman of Cromwell missing and he was spotted floating in about 5 feet of water.
