VAN WERT, OH. (WOWO): The Ohio State Highway Patrol’s Van Wert Post is investigating a fatal crash that happened just before 12:30 p.m., Monday afternoon.

76-year-old Carole Dunaway of Centerville, Ohio was driving on US 127 near Walnut Grove Church Road, in Van Wert County when she crossed the center line and hit a semi head on.  

Dunaway was pronounced dead on scene by the Van Wert County Coroner.  The driver of the semi was taken to the hospital for minor injuries.

The crash is still under investigation.
